Medical Exemption
For temporary medical conditions, please email or mail our office a statement from your physician specifically stating to excuse you from jury duty. This letter must be recent, from the current year, and be located on a letterhead. Our address can be found on your Summons under Option 3: Reply by mail. For permanent medical conditions, please email or mail our office a statement from your physician, specifically stating to excuse you from jury duty permanently. This letter must be recent, from the current year, and be located on a letterhead. Additionally, attach the completed form linked below and return it to our office along with the physician statement.
